Sending surveys

You can send surveys via text, email, or kiosk anytime from multiple places in ChiroUp EHR. 


Here's where you can access the request info screen: 

When scheduling a new appointment

When you schedule an appointment, you can request info using the Request info bar in the appointment panel.  

 

💡Pro tip: 

When you add a new patient from this appointment panel, your new patient surveys that you selected in your patient survey preferences  will automatically populate into the Request Info bar. 

 

 

When do appointment surveys send?

If you Request Info while booking the appointment, the survey requests will send alongside the appointment reminder (according to your reminder settings in Clinic Settings > Scheduling). 

If you schedule the appointment within the window in which an appointment reminder would send (24 hours is ChiroUp's default), then the surveys will send immediately upon booking the appointment. 

 

 

 
 

From the appointment panel

Click on any patient appointment to open the appointment panel and select Request Info.

 

When you request information from the appointment panel, if the appointment reminder has not yet been sent, you can choose to send the surveys now or schedule them to be sent with the reminder.

 
 

From the Communications detail module 

Navigate to the patient encounter and select the Communications detail module. 

 
 

 

 

Once you've opened the Request info screen, simply select the survey(s) you would like to send and select the destination. 

 

💡Pro tip: 

When you request surveys using one of the methods above, the survey request is linked to the patient's appointment. This allows the completed survey results to automatically populate into the patient's SOAP note.

If you cancel an appointment and schedule a new one, any surveys linked to the original appointment will not transfer to the new appointment. Instead, if a patient needs to reschedule, we recommend editing the original appointment with the new date and time. This preserves the survey link so the completed responses continue to auto-populate into the patient's SOAP note.
