ChiroUp offers two SOAP note templates: SOAP Wizard and Blank SOAP. The Blank SOAP template is structured with four empty free-text field boxes divided into Subjective (S), Objective (O), Assessment (A), and Plan (P) sections. Unlike the SOAP Wizard template, the Blank SOAP template does not pre-populate with any information. 

Here's how to create a SOAP note using the Blank SOAP template: 

1) After creating an encounter, you can switch templates by clicking the Template: drop-down below Encounter details. Select Blank SOAP.

2) Once you’ve switched the template to Blank SOAP, proceed to fill out details for each text box: Subjective, Objective, Assessment, and Plan. 

3) Optional: To add an additional section text box to the SOAP note, enter the section name in the field at the bottom of the screen and click Add Section.

4) Once you are satisfied with the SOAP note, click the Save & Sign icon (hint: its a pencil and note pad!). This will finalize the note and prevent further editing.

💡Pro Tip: 

If you need to edit a note that has already been signed, you can un-sign the note by clicking the pencil & note pad icon. This will allow you to make any necessary edits. Just remember to sign & save again!
