Types of surveys

ChiroUp enables you to send a variety of digital surveys to your patients in seconds! These surveys will help you effortlessly collect a variety of information (including patient demographics, subjective information, disability info, and more). You can send them via email, text, or kiosk.


Here's a lineup of available options: 

Subjective surveys

There are 3 subjective surveys in ChiroUp: Chief Complaint, Interim, and Re-exam

  • Chief Complaint Survey: 
    • Automatically sends to your patients 24 hours before their appointment (or 10 minutes before if the appointment is booked within the 24 hour window). 
    • An extensive and intuitive survey that will capture complete subjective information on your patient's chief complaint. 
    • Once your patient completes the CCS, their responses will populate directly into the subjective portion of your SOAP note. 
  • Interim Survey:
    • Completed by patients on subsequent visits (unless it is a re-exam visit). 
    • A shorter survey designed to capture information about the status of a patient's complaint. 
    • Only available upon check-in 
  • Re-exam: 
    • Completed by patients on their re-exam visits as set by the provider, or clinic default.
    • Less extensive than the CCS but more extensive than an interim survey - designed to capture more major details for a re-exam visit. 

The Chief Complaint is the only subject survey that you can manually send to a patient. (although it does also automatically send prior to an appointment for patients who do not have an active care plan or active complaints.) You cannot manually send the Interim or Re-exam surveys. 

The Interim and Re-exam surveys will be available upon check-in. The patient can either complete them when they check in on the kiosk, or, if the staff checks-in the appointment, the patient will receive a text or email (depending on their preferred contact method) to complete the interim or re-exam survey. Alternatively, these surveys can be completed on the provider's screen directly from the encounter.

  • Revised Oswestry Disability Index
  • Quick DASH
  • Lower Extremity Functional Scale (LEFS)
  • Back Bournemouth Questionnaire
  • Neck Bournemouth Questionnaire
  • Neck Disability Index (NDI)
  • Headache Disability Index

Functional disability surveys can be sent manually at anytime. In your Patient Survey Preferences you can set certain disability surveys to automatically and intuitively follow-up the CCS based on patient responses. 

 
 

Information requests

  • Photo ID: requests that the patient uploads a photo of their Photo ID
  • Insurance: requests the patient's insurance policy information and upload a photo of their insurance card
  • Patient Demographics: collects demographic information that will populate the patient's record.

Consent & acknowledgement forms

You have the option to upload and send your own consent and acknowledgment forms to your patients in ChiroUp.  We don't have any pre-loaded consent or acknowledgment forms in ChiroUp, but if you're interested in uploading and sending your own forms, check out this article for more details.  

 
 

Intake groups

You can send multiple surveys at once using intake groups. When sending a survey, simply select from the Basic Intake Group or the Advanced Intake Group.

  • Basic Intake Group: designed for simple presentations or re-activations of existing patients. 
  • Detailed Intake Group: designed for new patients.
