ChiroUp allows you to send an Insurance Survey to your patients to request their policy information. When a patient completes the survey, their policy will be automatically added to their profile. All you need to do is send the survey!
How to send an insurance survey
Sending an insurance survey is just like sending any other survey in ChiroUp. The quickest way to request surveys from your patients is to click Request Info from the patient's appointment panel.
Select Insurance and your delivery method— either text or email. (Insurance survey is only available to be completed via text or email because users will be asked to take a photo of their insurance cards!)

What does the insurance survey look like?
The insurance survey gathers all pertinent information including Payor name, Member ID, Plan/Group number, insurance type, relationship to the insured, and photos of the insurance card.
The Payors field will be a drop down of every payor within your Payors database.
Patients will have the option to add an additional policy or respond that they have no insurance coverage to report.

Where can I view insurance survey results?
When a patient responds to the insurance survey, their response will be recorded as a new policy within their patient record. It is recommended that staff check the reported policy information as patients can make mistakes and/or typos!
Here's how to view insurance policies:
1) From the patient's record, select the Insurance tab.
2) All patient insurance policies will show— including any reported by the survey or manually added by staff.
💡Pro tip:
When a patient completes an insurance survey, you will be alerted via the bell icon in the top right corner of your account! Simply click on the Insurance link and it will take you to the patient's record.
How can I view a patient's insurance card?
To view a patient's insurance card, simply click on the insurance policy from their patient record, and scroll down to the Card on File section.
You can replace, open, or download the file. To quickly view, click Open. A preview of the card will open up.
💡Pro tip:
Viewing the patient's insurance code photo is a great way to check that they entered their information (Member ID, payor name, etc) correctly!
Note!
The insurance survey only collects payor name, insurance type, member ID number, plan/group number, relationship to the insured, and photos of the insurance card.
The insurance survey does NOT collect any benefit information— staff members will need to enter insurance benefits.
