Types of digital surveys available in ChiroUp

ChiroUp enables you to send a variety of digital surveys to your patients in seconds! These surveys will help you effortlessly collect a variety of information (including patient demographics, subjective info on their chief complaint, disability info, and more). You can send them via email, text, or kiosk.

Consent & Acknowledgement Forms

You have the option to upload and send your own consent and acknowledgment forms to your patients in ChiroUp.  We don't have any pre-loaded consent or acknowledgment forms in ChiroUp, but if you're interested in uploading and sending your own forms, check out this article for more details

 
 

Intake Groups

You can send multiple surveys at once using intake groups. When sending a survey, simply select from the Basic Intake Group or the Advanced Intake Group. 

You can customize which digital surveys are included in these intake groups! Learn more here. 

  • Basic Intake Group: includes the Chief Complaint Survey, Health History, and Review of Systems. The basic group is designed for simple presentations or re-activations of existing patients. 
  • Detailed Intake Group: includes the Chief Complaint Survey, Patient Demographics, Family History, Health History, Lifestyle History, Medication History, Review of Systems, and Surgical History. The detailed group is designed for new patients.
