Re-evaluation Visits

At each visit, patients are prompted to complete one of 3 Subjective surveys. 

  1. Chief Complaint Survey for new patients, or patients without a complaint on file 
  2. Interim Survey for most follow-up visits after their New Patient visit. 
  3. Re-evaluation Survey for visits that fall on a patient's re-evaluation interval. 

This article will breakdown everything you need to know about Re-evaluation visits in ChiroUp and what fuels the Re-evaluation survey. 


⭐️ What is the Re-evaluation interval and how do I set it?

After a patient’s initial visit, where they complete the CCS, follow-up visits will typically prompt the patient to complete the Interim Survey. However, if the visit falls within the patient’s scheduled Re-evaluation interval, the patient will complete the Re-evaluation Survey instead.

The two surveys serve different purposes:

  • Interim Survey: Captures how the patient’s complaint is doing today compared to their last visit.
  • Re-evaluation Survey: Provides a broader update on the patient’s progress since starting care. It revisits key questions from the initial CCS, such as what best describes their pain, average pain severity, and palliative and provocative factors.

 

Set your default Re-evaluation interval in Clinical Skills > Condition Reference. 

Click the Ellipsis icon and then Edit Care Plan. 

 

Input your desired Re-evaluation Interval. In this example, patients will receive the Re-evaluation survey after every 5 visits. 

💡Pro Tip: Default Re-evaluation intervals are user-specific. This means that each provider in your clinic can set their own desired Re-evaluation interval!


⭐️ Can I change when specific patients will receive the Re-evaluation survey?

Yes! The default for your patients will follow your set Re-evaluation interval, however, you can change this for individual patients in the Schedule & Goals portion of your SOAP Wizard. 

Change the amount of visits until this patient gets the Re-evaluation survey:

⭐️ Does ChiroUp alert me when it's time for a Re-evaluation?

Yes! ChiroUp alerts your team when the current or next visit is expected to be a Re-evaluation. These alerts appear in both the Appointment and Encounter, so front desk staff and providers can easily see when a Re-evaluation is coming up.
