Every clinic has its own approach to caring for wellness and maintenance patients. Whether your workflow includes specific types of documentation, different kinds of intake processes, or specialized visit types, ChiroUp EHR provides the flexibility to standardize your clinic's unique workflow.

This guide covers the most frequently asked questions from clinics with wellness-focused patient populations and outlines best practices for efficiently managing maintenance care within the ChiroUp EHR.

What if the SOAP Wizard isn't compatible with how I take my wellness notes?

If you prefer complete control over your wellness documentation and do not want any information to automatically populate into your note, you can use ChiroUp's Blank SOAP Note Template

Benefits:

  • No pre-populated information
  • Four free-text sections for flexible documentation
  • Ideal for providers who prefer a fully customized note format

How to switch to the Blank SOAP Template:

  1. Create an encounter
  2. Under Encounter Details, select the Template drop-down.
  3. Choose Blank SOAP.

‼️ Important Notes:

  • All four sections must contain content before the note can be signed.
  • The Blank SOAP Template is not compatible with ChiroUp's automated Subjective Surveys
 
 
 

What if I want to indicate diagnosis codes for my wellness patients, but not use the rest of the SOAP Wizard? 

If you'd like to use the SOAP Wizard for only part of your documentation, you can take a hybrid approach by using the Wizard where it adds value and treating the SOAP Output as a flexible, free-text note for the remaining sections.

Benefits:

  • Use the SOAP Wizard only for the sections you need (such as diagnosis coding).
  • Maintain flexibility with free-text documentation for the rest of the note.
  • Continue leveraging ChiroUp's diagnosis and coding tools without being locked into the full SOAP workflow.

Getting Started:

The SOAP Wizard requires an active complaint to unlock the Assessment, Objective, and Plan sections. You can add a complaint by:

  • Having the patient complete a Chief Complaint Survey, or
  • Clicking the "+" next to Manage Complaints and completing the complaint via My Screen.

Example Workflow for Wellness Patients:

  1. Add a complaint to activate the SOAP Wizard.
  2. Use the Assessment section to document the patient's condition and select the appropriate diagnosis code(s).

 

  1. Click the orange SOAP button to generate the SOAP Output.
  2. Use the remaining free-text sections to document any additional information in your preferred format.

 

‼️ Important Note:

All sections of the note must contain content before the encounter can be signed.

 
 
 

What if I don’t want my wellness patients to receive ANY automated surveys? 

If you do not want wellness patients to receive any automated surveys, you can disable survey automation for that treatment type by navigating to Clinic Settings > Disciplines and enabling the option to suppress automated surveys.

Benefits:

  • Prevents wellness patients from receiving unnecessary surveys.
  • Creates a streamlined patient experience for maintenance care visits.
  • Ensures no automated surveys, including the Chief Complaint Survey, are sent when patients are scheduled under that treatment type.
 
 

What if I want patients to complete the Chief Complaint Survey on their first visit, but I don't want patients to receive any surveys for subsequent visits?

If you're using the SOAP Wizard for maintenance patients and want the initial complaint to auto-populate, keep Suppress Automated Surveys toggled OFF in that treatment type. This ensures the Chief Complaint Survey is sent before the first visit, allowing subjective information to automatically populate into the encounter and streamline documentation.

For ongoing care, if you want to prevent additional surveys (such as interim or re-exam surveys), you can simply uncheck “Send Surveys” within the patient’s Track settings.

Benefits:

  • Captures structured subjective data automatically at the first visit
  • Reduces manual documentation by pre-populating encounter information
  • Gives you control over whether follow-up surveys are sent during maintenance care
