What are Tracks?

What is a patient Track? 

A Track acts like a dedicated folder for a patient’s SOAP notes and care plan related to a specific complaint or set of related issues. For example, if a patient has back and rib pain from a car accident, their doctor will have a Track containing all relevant notes.

If the same patient sees a massage therapist for muscle pain, the therapist will have a separate Track with their own notes. This ensures each provider accesses only the information relevant to their treatment, keeping records organized and care streamlined.


What is the purpose of Tracks? 

Patient Tracks automatically organize patient complaints, SOAP notes, and care plans across visits, ensuring consistent care—even with different providers. They also offer flexibility for multi-provider clinics, ensuring each provider accesses only the relevant information. Tracks streamline treatment documentation and trigger surveys at the right times, keeping care organized and efficient.


How are Tracks created?

Upon release, all active patient complaints will be consolidated into a single tracking system— no action required by your clinic!

For new patients with no existing tracks, a track is automatically created when they complete the Chief Complaint Survey. For multi-segment appointments, each provider on the appointment will have their own dedicated track linked to the complaint.

For existing patients with existing tracks, you can create a new track by clicking the Create track on CCS submission check box. This will send a Chief Complaint Survey to the patient and create a new track with that associated complaint. 

You can also create a track for a patient directly from their record in the manage complaints window.
