ChiroUp’s Voice to Chart feature helps streamline documentation by securely recording patient interactions and generating a subjective synopsis for your SOAP note using AI assistance. This article breaks down how to properly use this tool from start to finish.
Your ChiroUp EHR subscription includes 100 free minutes each month. If you want more, you can upgrade your subscription to include ChiroUp's Voice to Chart, which provides an additional 1,500 minutes per month.
Note: Voice to Chart is only available when using the SOAP Wizard. Voice to Chart is not compatible with the Blank SOAP template.
Here's how to use Voice to Chart:
- Navigate to the patient's encounter and click the microphone icon to start recording.

2. During the first recording attempt in a patient’s encounter, the system will prompt you to confirm that verbal consent to record has been obtained. Once you have obtained verbal consent, click Consent Obtained to proceed with recording.
No additional consent is required for future visits with the same patient.

💡 Pro Tip: For added protection, consider obtaining written consent. See our guide on Consent to Visit Recording. You can upload this sample consent form to your ChiroUp Forms Library to send out to patients.
⚠️ Important: Always confirm a patient’s preference before initiating a recording. This ensures compliance with consent but does not replace the need for proper verbal and/or written consent where required.
3. After clicking Record and obtaining consent, Voice to Chart will begin recording your conversation with the patient. The summary of this conversation will input into the Subjective section of your SOAP Output.

💡Pro Tips for recording Voice to Chart:
- For best accuracy, record primarily during the subjective portion of the visit— when discussing the patient's complaints. While continuous recording is allowed, avoiding exam and treatment chatter enhances output performance and saves recording minutes. Additionally, the summary output will only populate into the subjective portion of your note.
- Make sure your device's microphone is unobstructed so that Voice to Chart can capture the entire conversation. If your computer's built-in microphone does not efficiently pick up your audio, we recommend purchasing a microphone attachment for your computer.
Voice to Chart can’t interpret gestures or vague statements. If a patient points or gives unclear answers, be sure to restate their comments clearly.
For example:
- If a patient points to their right shoulder and says, “This feels better,” respond with, “So your right shoulder is feeling better.” Or if they wipe their hand over their back and thigh and say, “The pain kind of travels here,” clarify by saying, “So the discomfort travels from your lower back into your right hip and thigh.”
- When answers are vague or ambiguous, summarize with something like, “To confirm, your right-sided neck pain has gradually improved over the past week.” These clarifications not only help the Voice to Chart generate a more accurate summary but also ensure better communication with your patient.
Voice to Chart will automatically filter out any non-clinical conversation. Discussions about weekend plans, personal stories, and similar will be excluded from the output. If the recorded conversation only contains personal anecdotes, the Voice to Chart output will generate an output along the lines of “Please provide a transcription…"

4. When you are finished recording the visit, click the green check mark to complete the recording and submit for processing.
You can also click the red Pause button to pause and resume recording.

Proceed with taking your note, documenting information in the Assessment, Objective, Plan, and Schedule & Goals sections. During this time, Voice to Chart will be busy generating the output. It may take a few minutes for the output to generate depending on recording length.
✍️ Please note that you can navigate through your note, and the recording will continue. To complete the recording, you must click the green check mark.
5. When done with your note, navigate to the SOAP Output and review the processed recording summary in the VoiceNote Summary - Subjective box.
Just like the other fields in your SOAP Output, the Voice to Chart Summary is free text and editable! If you'd like to delete the entry, simply delete the output text.

✍️ Remember: You remain responsible for verifying accuracy. You can unsign and edit the note at any time, including changes to generated content.
💡Pro Tip:
Once processing finishes, you’ll find the full audio and word-for-word transcript in the Recordings tab at the bottom of the SOAP Wizard. This version is just the raw recording used to generate the visit summary—it’s not the one you'll import into the note. The actual summary you’ll use appears in the SOAP Preview, where you can review, edit, or accept it before finalizing.

Click on the Play icon to listen back to a recording, or click the beginning of the transcript preview to review and edit the full transcript.

The recording and transcript are only available before signing the note. After you initially sign the note, you will not be able to access the recording or transcript.
Security & Compliance
ChiroUp’s Voice to Chart features are designed with patient privacy and data security in mind. All audio recordings, transcriptions, and AI-generated summaries are processed through encrypted, HIPAA-compliant systems. Recordings and transcripts are stored temporarily and handled in accordance with strict privacy standards to ensure the confidentiality of protected health information (PHI).
🔒 Rest Assured: No data is used for AI training purposes, and all third-party services involved operate under valid Business Associate Agreements (BAAs) where required.
To ensure accountability and compliance, all access to recordings and transcripts will be logged, including timestamp, user ID, and access history.
