Patients self-report complaints in ChiroUp by completing the Chief Complaint Survey. Using the body picker, patients identify the area causing them discomfort and provide details about their complaint. This information is then carried into the patient’s SOAP Note, helping streamline the documentation process.
You can customize how patients report their complaints by controlling two settings:
- Unrelated regions: Choose whether patients can include multiple, unrelated body regions within a single complaint.
- Number of complaints: Choose whether patients can report multiple separate complaints on the same survey.
This article explains how each setting works and how to configure them to fit your clinic’s workflow.
💫 Unrelated Regions
By default, patients completing the Chief Complaint Survey can select multiple unrelated regions on the Body Picker at once. When they do, ChiroUp combines the selected regions into one complaint in the SOAP Note.
💡 Unrelated regions are selected body regions that do not touch or share a border with another selected region.

If you prefer patients to select only adjacent regions within a single complaint, you can enable a survey setting that prevents patients from selecting unrelated regions on the Chief Complaint Survey.
Navigate to the profile icon in the top right corner and select Clinic settings > Survey Preferences.
Scroll down to Chief Complaint Survey.
- Toggle this ON to prevent patients from selecting unrelated regions.
- Toggle this OFF to allow patients to select related regions.

💡Pro tip:
When this setting is toggled ON, patients will be alerted that they have selected multiple unrelated regions and will have to Try again:

When this setting is toggled OFF, patients will still be alerted of unrelated selections, but they will have the option to Continued or Try again:

💫 Number of Complaints:
At the end of the Chief Complaint Survey, patients will be asked if they have any additional complaints to report. If they respond Yes, then ChiroUp will prompt them to complete the survey again in order to report their additional complaint.
By default, patients can report up to 2 separate complaints on the Chief Complaint Survey. Here's what it looks like when a patient reports multiple separate complaints:

Control how many separate complaints a patient can report on their Chief Complaint Survey by customizing the survey.
In your Custom Survey Settings, configure the Max Complaints you want to allow patients to report when completing the CCS.
- You can customize other parts of the Chief Complaint Survey, but if you only want to change the maximum number of complaints a patient can report, simply update this field. Then, in your Survey Preferences, select your customized version of the survey to replace the system default.
