After you've finished building your survey, it's time to configure your Survey Settings.
This article will walk you through how to customize your survey's display, behavior, and SOAP Output settings before making it available to patients.
⭐️ Here's where to access Custom Survey Settings:

Click Settings on the left side of your Custom Survey View.

⭐️ Customizing Basic Info
Below is an outline of the Basic Info fields for custom surveys. This is where you will label the survey and define behavior.

- Enter a Survey Name. This internal name is visible only to your team when selecting and managing surveys.
- Enter a Display Title. This is the title patients see when completing the survey.
- Optionally, add a Subtitle to provide additional context on the survey's title screen.
- Assign a Category to organize your survey within the Custom Surveys library.
- Choose how the Survey Behavior:
- Standard Survey: Functions as a standalone survey that can be sent to patients like any other intake survey.
- Behave as CCS, Abbreviated CCS, Interim, or Re-Examination: Replaces the default survey for the selected workflow. Only one custom survey can be active for each survey type at a time. For example, you can have only one survey designated as your active CCS.



⭐️ Customizing Patient Messaging
Customize the messages patients see at the beginning and ending of their surveys!
- Enter a Start Message for patients to see before beginning the survey.
- Enter a Completion Message that will be displayed at the conclusion of the survey.



⭐️ Subjective Output Template
When you request a survey from the appointment panel, the patient's responses automatically populate the Subjective section of the your SOAP output.
This section lets you customize exactly how those survey responses are formatted in the SOAP output.
By default, ChiroUp displays each survey question followed by the patient's answer. See example below.
You can skip this section unless you'd like to customize how the survey responses appear in the SOAP output.

To further configure your SOAP Output Template for a survey, use merge fields to automatically insert the survey title, questions, and patient responses into your template.

| {id:TemplateName} | Inserts a patient's answer to the question using the question's Template Name instead of its question number. This is useful if you reorder questions later, since Template Names remain the same. | 5/10 (the answer to the templated question) |
Recommended Workflow:
- Click Insert Default Template to generate a starting template.
- Customize the wording as needed.
- Wrap each question and answer in {#aN}...{/aN} so unanswered questions are automatically omitted from the SOAP note.
- Review the Default Output Preview to verify the final format before saving.

If the patient skipped Question 3, that line would be omitted automatically.
⭐️ Survey Library
You can share your custom survey with the ChiroUp Community!
Toggle ON Publish to survey library to make your survey available to other ChiroUp clinics.
Other clinics will be able to import a copy of your survey into their own clinic for use.
Choose whether to show your clinic name in Survey Library and add a description to associate with it.

💡Are you editing a Chief Complaint, Abbreviated Chief Complaint, Interim, or Re-Exam survey?
If your goal is to replace the default survey that is automatically sent to patients, you'll need to update the behavior settings on your customized version of the survey.
To do this:
- Open your survey.
- Under Basic Info, select Survey Behavior.
- Choose the survey type you want this survey to replace.
Note: Only one survey can be assigned to each survey behavior at a time. For example, whichever survey is set to Behave as Chief Complaint Survey (CCS) will be the survey automatically sent to new patients. The same applies to the Abbreviated CCS, Reevaluation, and Interim surveys.

To manage which surveys are currently assigned to each survey type, navigate to Clinic Settings > Survey Preferences. Here, you can view which surveys are designated as your Chief Complaint, Abbreviated Chief Complaint, Interim, and Reevaluation surveys.
