Building & Editing Custom Surveys

ChiroUp surveys are built on research-backed best practices to help you consistently collect meaningful patient information. At the same time, every clinic works a little differently, so Custom Surveys give you the flexibility to tailor questions and structure to match your workflow.

This article will walk you through everything you need to know to design and structure your survey.

📍Add a Survey to Customize

1. Access Custom Surveys from the Admin tab in your navigation panel. 

 

2. Click the + Create your First Survey button.

3. Choose what kind of survey you want to add: 

  • Build from scratch: Start with a blank survey and create all your own questions and answer options.
  • Browse surveys library: Browse and import ChiroUp default surveys or customized surveys published by other clinics. 

 

4. If you choose to Build from scratch, you're ready to move on to Building & Editing Surveys! 
 

If you choose to Browse surveys library, you can select which survey you want to import. Click Import on the survey you want to customize. After importing, you're ready to move on to Building & Editing Surveys!

Other clinics in the ChiroUp community can choose to make their own surveys public and available for your to import to use in your own clinic. 

You can decide to publish your own versions of surveys to the community in your Custom Survey Settings.

🌟 Types of Questions

Select from the question type here: 

Here's a breakdown of each question type: 

Multiple Choice (Single): Select one answer from a list of options.

Multiple Choice (Multiple): Select one or more answers from a list of options.

Slider / Scale: Select a value along a defined range (e.g., 0–10 or low–high).

Short Text: Enter a brief response, typically one sentence or phrase.

Long Text: Enter a detailed response with multiple sentences or paragraphs.

Date: Select a specific calendar date.

Question Group: A section that includes multiple questions on the same page

 
 

🌟 Building & Editing Questions

Start by selecting the question you want to edit, or click Add Question to add a brand-new question. 

Next, here's how you can customize the question:

  1. Select your Question Type using the drop-down
  2. Enter your Question Prompt- this is the question that patients will see
  3. Optionally, add a Section Label that will help organize related questions while building your survey. Assign the same section label to related questions to make your survey easier to manage in the editor. You'll see the section label in your master list of questions (see image below).
  4. Add a Template Name to make it easier to reference this question when building your SOAP Output later in Survey Settings. Instead of relying on a generic question number, you can use a consistent name as a placeholder. This keeps your SOAP Output organized and prevents issues if you reorder questions later. 
  5. Indicate if this question Requires an Answer, or if patients can bypass. 

 

Manage your question list using the icons alongside each question:

  • Reorder questions by clicking the three lines next to each number and dragging.
  • Copy or duplicate questions by clicking the copy icon.
  • Delete questions by clicking the trash icon
 
 

🌟 Building & Editing Answers

To configure answer options, switch to the Configure tab. 

Here, you can configure the available answer options based on the question type.

 

Multiple Choice (Single & Multiple): 

  • Enter, reorder, add, or delete answer options
  • Toggle Include “Other” to include a free-text “Other” answer option

 

Slider/Scale:

  • Set the Min and Max values for the slider
  • Define the Interval (the step increments between values)
  • Label the Left (minimum value)
  • Label the Right (maximum value)
  • Set the Initial Value (default position when the question loads)
  • Choose whether to display pain scale icons (sad/happy faces)

 

Short Text:

  • Select whether patient answers should be Text or Numbers
  • Optionally include a Placeholder ("hint" text shown in the field before the patient types)

 

Long Text:

  • Optionally include a Placeholder ("hint" text shown in the field before the patient types)

 

Date:

Date questions will display a calendar view where patients can select a date.

  • Enable Allow estimated dates to let patients mark a date as exact or approximate
  • Enable Future dates only to restrict selection to future dates only
  • Enable Past dates only to restrict selection to past dates only

 

Question Group: 

Question groups allow you to display multiple questions of any type on the same page of the survey. 

  • Enter a Group Heading that will appear at the top of the Question Group page.
  • Proceed with entering additional questions of any type and configuring their answers. 

 

 
 

🌟 How do I add Conditional Logic (AND/OR rules)?

Add conditional logic when you want a question to appear only in certain situations, based on how a patient answers earlier questions. This helps you create more personalized and relevant surveys without showing unnecessary questions.

Most surveys use simple logic with either AND rules or OR rules.

  • Use AND rules when all conditions must be true for the question to appear.
  • Use OR rules when any one condition is enough within that group.

 

Here is an example of an AND rule: 

  • Question 3 will be prompted when the patient responds “Yes” to Question 1 AND “Massage Therapy” to Question 2. 

 

Here is an example of an OR rule: 

  • Question 2 will be prompted when the answer to Question 1 is “Employment-related Accident” OR “Auto Accident”

 

Mixing AND and OR rules?

When combining AND and OR rules, both groups of conditions are evaluated together. 

The question will only display when all AND conditions are met AND at least one OR condition is met.

 
 

🚀 Saving Custom Surveys

🚀 Saving your Survey

Before saving your custom survey, review the three survey settings below to ensure they're configured correctly. Navigate to Settings to verify each one, then click Save when you're finished.


1. Survey Name

  • Enter a Survey name. This is the name your team will see when sending surveys to patients. It's also the name patients will see if you don't enter an alternate Display Name.

2. Category

  • Assign a category to your survey. This category determines how the survey is organized in the Survey Library.

3. Survey Behavior

  • Indicate what type of survey this is and how it should behave. For example, if you are editing the Chief Complaint Survey, indicate that it should behave as a CCS. 
    • đź’ˇPro Tip: When you indicate that a survey should behave as a CCS, ChiroUp will automatically add the body picker first question. 
  • If you want this survey to replace the default CCS/interim/re-exam that automatically sends to patients, make sure to update this in Clinic Settings > Survey Preferences > Custom Survey Replacements. Learn more in the Pro-Tip below! 
    • Note: Only one survey can be assigned to each survey behavior at a time. For example, whichever survey is set to Behave as Chief Complaint Survey (CCS) in Clinic Settings > Survey Preferences will be the survey automatically sent to new patients. The same applies to the Abbreviated CCS, Reevaluation, and Interim surveys.

You'll see all of your saved Custom Surveys from your main Custom Surveys screen. 

💡Pro Tip: 

To manage which surveys are currently assigned to each survey type, navigate to Clinic Settings > Survey Preferences. Here, you can view which surveys are designated as your Chief Complaint, Abbreviated Chief Complaint, Interim, and Reevaluation surveys.

đź‘€ Preview Your Survey

Before assigning your Custom Survey to patients, preview it to ensure your questions, answer options, and conditional logic are configured exactly as intended.

Click Preview to open the survey preview screen. From there, you can choose between two preview modes:

  • Act as Patient: Experience the survey exactly as a patient would. This preview follows all Conditional Logic Rules, so questions will appear or be skipped based on your responses.
  • See All Questions: View every question in the survey, regardless of any Conditional Logic Rules. This is helpful for reviewing your entire survey at once.

Click the Next button to proceed through the survey.
