Create fillable forms tailored to your practice's workflow. Custom Forms allow you to turn the forms your office already uses into interactive forms to collect information, capture electronic signatures, and save completed forms directly to Patient Documents.
Custom Forms support both patient-facing and internal workflows. Send forms to patients to complete, or create staff-managed forms for internal use or to customize before sending.

📍 Create a Customizable Form
1. Access Custom Forms from the Admin tab in your navigation panel.

2. Click the + Create your First Form button.

3. Choose how you want to add your Custom Form
- Build from scratch: Start with a blank page and create all your own text and fields.
- Upload PDF: Upload an existing PDF that your office already uses and place fillable fields on top of it.
- Browse forms library: Browse and import forms created by other ChiroUp clinics to use in your own office.
- Import from Legacy Library: Use forms you've previously uploaded to your ChiroUp account under Practice Resources > Forms Library > Consents & Acknowledgement surveys.

- If you choose Build from scratch, you are ready to proceed to the next section: Building Custom Forms.
- If you chose to Upload PDF, you'll be prompted to upload your PDF from your computer's file manager.

- If you chose to Browse Forms Library, click Import on the survey you would like to use and edit for your own clinic.

- If you choose to Import from Legacy Library, select from your existing consents to import.
- We recommend leaving the Also remove them from the old Consents list box checked to prevent duplicate versions of the same consent from existing in your ChiroUp account. Once you've finished building it as a Custom Form, you can begin sending it to patients from there.

🌟 Designing Custom Forms
🌟 Adding, Copying, & Deleting Fields
Select a field type from the Elements list and place it directly on your form wherever information needs to be collected.

Easily duplicate an existing field by selecting it and clicking the Copy icon, or by using your standard copy and paste keyboard shortcuts. (Ctrl+C / Ctrl+P for Windows, Cmd+C / Cmd+P for Mac)

Delete fields using the trash icon.

🌟 Customizing Element Behavior
Select any element to customize its settings using the options on the right side of the screen.
🌟 Field:

- Label: Assign a text label to the appear at the top of the field.

- Placeholder: Add "hint" text shown in the field before input is added.

Default value: Enter default text to pre-fill the field. This can be changed by the person completing the form.
- Use the +Insert auto-fill token drop-down to have the default value populated based on automated fields.
- Example: Select {{patient.legalFirstName}} {{patient.legalLastName}} to default the field with the first and last name on the patient's record.
- (Note: The default value populates as the form is being completed. You won't see the default value entered in the Design view.)


- Required: Requires the element to be completed. Adds red required star to Label.

Completed by: Indicate whether the field should be completed by the patient or by the user.
- If a form contains User-completed fields, it must be generated from Patient Documents. This allows your clinic staff to enter the required information before sending or completing the form. Forms with User-completed fields cannot be sent through the standard Request Info button.

Only show conditionally: Opt to only display this field when conditions of another field are met.
- Example: A form asks a Yes/No question, and if the patient selects Yes, you want them to provide additional details. You can configure the follow-up field to show only when Yes is checked.
🌟 Style:

Field Border & Filed Background: Designate a colored border or background for certain field types.
- Note: Note all field types are compatible with Styling.
🌟 Layout:

Position & Size: Fine-tune the placement and dimensions of a selected field to align it exactly where you need it.
- X and Y: Use the arrows that appear when you hover over each box to move the selected field left, right, up, or down.
- Width and Height: Use the arrows that appear when you hover over each box to increase or decrease the field's width and height.
🌟 Types of Fields
Single Line Field: Enter short text responses

Multi-Line Field: Enter long text responses

Date: Pick a date from the calendar, or default the field to Today's date


Number/currency: Enter a numeric value. Optionally designate as currency to display dollar sign.


Checkbox: Check a box to acknowledge a selection.

Radio group: Select from a list of options.

Dropdown field: Select from a drop down of options.

Auto populate Token: Select from an extensive list of Auto-Populate Tokens to automatically fill form fields with information from ChiroUp. Unlike fillable fields, Auto-Populate Tokens display existing information and cannot be edited by patients or staff.

Switch to the Settings tab.

🛠️ Details:
- Form Name: This is how the form is labeled in your Custom Forms Library

Display Name: This is the name used when requesting or generating the form. It is also the name patients see when completing the form.
- When no Display Name is saved, then the Form Name will appear.
- Subtitle: A short description shown below the Display Name.
- Category: This is how your Form is categorized in your Custom Forms Library.

🛠️ Delivery & Storage:

Destination folder: Select the folder in Patient Documents where completed forms will be stored.
Send to patient: Controls whether the form can be sent to patients. Turn this setting OFF for forms intended only for internal use or staff completion and for any forms you're still working on.
🛠️ Form library:

You can share your Custom Form with the ChiroUp Community!
Toggle ON Publish to form library to make your form available to other ChiroUp clinics.
Other clinics will be able to import a copy of your form into their own clinic for use.
Choose whether to show your clinic name in Form Library and add a description to associate with it.

The Preview screen shows you what patients will see when they complete the Form.
