You can upload the results of any completed survey directly to the SOAP Output for an encounter, even if the survey wasn’t originally requested for that specific visit.

Here's how to upload survey results into your SOAP Output: 

Navigate to the SOAP Output by clicking the orange SOAP button. 

Then, click the cloud icon button. Doing so will open a window with all completed surveys in the patient's chart. 

Select the survey(s) you'd like to upload and click Import Surveys. 

The survey results will be uploaded into your SOAP Note output.
