ChiroUp will automatically prompt a Subjective Survey for each patient visit - either the Chief Complaint Survey, Interim, or Re-Exam surveys. The patient's response to these surveys will auto-populate the Subjective portion of your note.
You can edit both the regions selected on the body picker and the survey question responses.
Here's how to edit subjective survey responses:
Navigate to the Subjective portion of the patient's SOAP Note.

To edit any response, click on the green-highlighted text. This will open response options to update. Make sure to confirm any changes by clicking Done.

Here's how to edit the region of complaint on the body picker:
To edit the selected body picker region, click the pencil icon next to the body picker. Updating the region of complaint will also update the Complaint Name.
Please note that you can only edit the body picker region of complaint on the initial CCS response note.

Here's how to clear all survey responses for an encounter:
Click the Eraser icon.

Confirm that you would like to clear any newly reported complaints and/or interim and re-exam responses. This action cannot be undone.
Any complaints reported on a CCS for this visit will be deleted. After clearing responses, you can add a new complaint from My Screen or redo the interim or re-exam survey.

💡Pro Tip:
Edits made to subjective responses will only apply within the SOAP Wizard. These changes will not update the original responses stored in the patient’s chart.
