Adding Conditions and ICD Codes in the SOAP Wizard

Adding a Condition

1) Click the "+ Conditions" button to open the condition menu.

2) Select one or more conditions by clicking on them. A selected condition will appear bold with a green checkmark. Click again to deselect.

3) Use the regional headers at the top to filter the list, or use the search box to search for a specific condition by name.

4) Use the hotkeys on the right of the search bar to quickly select joint dysfunction. Learn more about setting hot keys. Hyperlink

5) When finished, click Close, located at the bottom of the menu.

Selected conditions will populate the associated objective tests and treatment detail options for that condition, streamlining your note-taking process.

💡Pro-Tip:

To favorite a condition for a given region, click the heart icon next to it. Favorited conditions will appear at the top of their regional menu/ list for all future selections. (Clicking on the heart icon a second time will unfavorite that condition) Users can also favorite a condition from the Condition Reference library.

 

 

 

Adding Associated ICD Codes

💡Pro-Tip:

Selecting an associated ICD code is optional, but when selected, the ICD code will auto-populate in the subsequent billing module. Users can customize pre-set ICD codes from their Condition Reference library.

 

 

After adding conditions and clicking Close, your selected condition(s) will appear in the assessment module.

If you’ve favorited an ICD code for that condition, it will automatically populate in the ICD field to the right of the condition name. ICD codes can only be favorited within the Assessment Wizard.

1) Click the '+' (Add ICD) to view available ICD codes associated with the Condition and select the appropriate one. If no ICD codes are associated with the Condition, the ICD field will be blank, and you'll need to search for a code by selecting 'Add new reference code.' Note that adding a new reference code will only apply to this encounter and will not update the Condition Reference.

2) To add a unique diagnosis that is not a Condition in the ChiroUp system, click on the Additional/ICD Codes field at the bottom of the page and start typing to search for the ICD code, select the code, and click the Green Arrow to the right of the ICD field to import it onto your diagnosis list.

 

Rearranging and Removing Diagnoses/ICD Codes

Conditions and their associated ICD codes can be rearranged by dragging and dropping the respective boxes in the desired order for how they will appear on the subsequent insurance claim form.

1) To remove an associated ICD code from a condition, click the trash can icon next to the ICD code. The ICD code will be removed, and the diagnosis will not appear on the subsequent insurance claim form. However, the condition will still auto-populate the associated SOAP wizard objective tests and plan treatments for that condition. 

*Note:

This option is ideal for functional diagnoses like "upper-crossed syndrome," where you want to document objective findings and support exercise recommendations in the SOAP note without listing it as an official claim form diagnosis.

 

 

2) To delete the entire condition and its associated diagnoses, click the X icon on the right side of the condition bar.

 

Accessing the ChiroUp Condition Reference

1) Click on the Play icon within the condition field to open the ChiroUp Condition Reference, which provides a detailed synopsis of current best practices and allows you to customize any associated tests, treatments, or diagnoses. (Hyperlink to customizations)
