There are three methods available for delivering digital surveys to patients for completion:

1) Email - A link will be sent to the patient's email. After opening the link, the patient will be prompted to enter a verification code sent to their email. After authentication, the patient will have access to the digital survey(s).

2) Text message - A link will be sent to the patient's mobile number. After opening the link, the patient will be prompted to enter a verification code sent to their mobile number. After authentication, the patient will have access to the digital survey(s).  

3) Kiosk - The patient will be prompted to enter their mobile number to "log in" to the kiosk. Any outstanding digital surveys will be available to complete. 

*Note: Patients are required to enter a verification code to access surveys and consent forms sent via text message or email. The code will be either emailed or texted to the patient. ChiroUp has implemented this measure as another way to verify patient identity and ensure the privacy and security of their protected information.
