If you decide to discontinue ChiroUp, we can provide a standard electronic export of your clinical records and related documentation, including SOAP notes, condition and exercise reports, uploaded chart documents, consents, and acknowledgments. Export files are delivered securely to an authorized representative of the account and are typically fulfilled within five business days.
Other information, including patient demographics, appointments, billing-related records, and reporting data, may be downloaded at any time through the reporting tools within your account.
One standard electronic record export is provided at no cost. In the unlikely event that a subscriber needs a second standard export within 12 months of discontinuation, we will also provide it at no cost. Custom exports, format conversions, expedited requests, or requests requiring significant manual processing may be subject to an administrative fee proportionate to the time and resources required to fulfill the request.
Requests for data exports may require verification of account ownership or authorization. Certain third-party integrations, custom settings, audit logs, or non-standard data elements may not be included in the standard export package.
While ChiroUp currently maintains historical records after account discontinuation, subscribers are encouraged to download and securely retain their own records to ensure long-term accessibility and compliance with applicable retention requirements.
